We are due to have some roof repairs carried out in the next couple of months and I am working to ensure everything is in place in relation to CDM Regs. This is the first time I've been involved in a project like this as a client and I'm wondering if anyone has templates (or suggested wording) that they wouldn't mind sharing for appointing Principal Designer and Principal Contractors as required under Regulation 5? There will only be one contractor who will be undertaking both these roles so no need for any additional appointments.

You do not really need a template. Most templates just duplicate the regulations and can be full of waffle. I would just keep it brief, using emails and a headed letter, and get them to reply.
If you are entering a formal contract check it has been amended to show dual roles. This will also act as confirmation they have accepted the both roles.

I agree with Alfasev.
Your contract should state who the duty holders are and it is generally good practice (IMO) to get the PC to name the duty holders in their CPP.
Again I am in agreement with Alfasev, make sure you have a clear contract which clearly states that the one contractor will be PC and PD

Where the appointments letters are purely for that purpose (i.e. don't contain any other contractual details) would you normally ask the organisation being appointed to sign in acceptance of the appointment?

It’s good practice to get something back from them. Trust me if it all goes wrong they will try and worm their way out in court.
