Speak to a company called "IBC". They definately do Accident Investigation and possobly Report Writing.

Posted By Mark Preston HSE's HELA has a site for training Local Authority inspectors that has a realistic accident investigation case study that features excellent guidance. Though a good deal of the material involves enforcement judgements, it is nonetheless a valuable resource for anyone interested in accident investigation. (It might also give you an insight into what to expect when an inspector calls). It's one of the best on-line training resources I've seen recently and it's free! Go to www.e-hazard.org.uk
