Speak to a company called "IBC". They definately do Accident Investigation and possobly Report Writing.

There are a number of courses offered by IOSH - if you check the Professional Development Courses 2001 brochure, pages 11, 12, 15 and 39, may be of help.

HSE's HELA has a site for training Local Authority inspectors that has a realistic accident investigation case study that features excellent guidance. Though a good deal of the material involves enforcement judgements, it is nonetheless a valuable resource for anyone interested in accident investigation. (It might also give you an insight into what to expect when an inspector calls). It's one of the best on-line training resources I've seen recently and it's free!
