Would anybody have a questionnaire that would aid carrying out risk assessments with pregnant workers in an office and call centre environment?

Can I suggest that you get hold of a copy of the HSE's publication "New and Expectant Mothers at Work" - a guide for employers -
ISBN 0-7176-0826-3.

It is an excellent publication & should give you all the information you need.
