Abdul,
a risk assessment is a process carried out by the workers before and throughout their activities. What you are looking for is a set of controls. Try this policy document for a starting point, it has some useful information in it,

What sort of working environment would this be for? eg Healthcare, Housing management, demolition and construction.
Sharps procedures are relevant in all these but the risks are different and therefore the controls will need to reflect.
There is a lot of guidance on line from NHS / Construction bodies and Local Authorities - Torbay Council I recall had a good one on line.

Abdul,
I'd be most concerned if you used a 'generic' assessment. Each location would need to be assessed and from this a RA built up rather than using a one fits all assessment.

Hi Abdul If you want a good risk assessment you will need to carry out a risk assessment at the site with the operatives so that all the risks are visible the operators will make you aware of other risks that they have come across.The alternative to this is to get someone to come in and do the assessment for you no one can send you one as they will not be familiar with the area that you work in. remember you or they must be competent to carry out such an assessment

I have a generic one for Blood Borne Viruses which I used in my last job in Social Services but now use in the PCT. This was approved in conjunction with Occupational Health
