Posted By Rob M Hi all, I am in the process of educating the workforce to formally report any defects. I am considering a duplicate book type of system, or a single sheet, not sure yet. Does anyone have a good system they are prepared to share? Thanks, Rob.

Posted By Gus A useful explanation www.quarrysafe.com/QHSMS...cidents.Issue1.08.06.pdf - the cards / books is the easy bit... follow up and publication of actions is much harder but just as critical. The ideas should be to get people to spot it- fix it - and report it otherwise it becomes a very long to do list!! Gus
