Hi all, We are looking to develop a questionnaire to provide to a sample of our workforce to gain a "snap shot" of their opinon on the current safety culture within our organisation and their level of knowledge / understanding of the content of our safety management system. Im struggling to come up with a format / content for the questionnaire and was wondering if anyone would be able to shed some light / provide an example of a safety culture questionnaire that I could use as inspiration?!
