Recently we introduced a “Don’t Walk By” Scheme which was launched in September 2014, due to it being identified that in the previous twelve months the Company had only received three reports.

Since the re-launch of the scheme and a positive marketing campaign by the SHE team, the Company has seen over 60 “Don’t Walk By” reports submitted. The “Don’t Walk By” issues being reported can range from estates issues which need to be addressed such as trip hazards to more serious issues which have helped avoid life threatening accidents. In particular one ”Don’t Walk By” report has resulted in preventative action across our Company.

A member of staff reported that he felt the platform boards utilised on a piece of Aircraft Staging were trip hazards and did not comply with the Working at Height Regulations for the construction of Scaffolding Towers.

Following liaison with the SHE team and a closer examination of the original drawings it became evident that there were parts missing from the construction of the staging which were considered to be a Scaffold Tower under the working at height regulations due to both its method of construction and it being able to be altered dependent on the work carried out.

The staging was immediately quarantined and withdrawn from use to ensure it was not utilised by the section; this then led to the SHE Team reviewing the last time training had been received by the team who were required to inspect the tower.

As a result of these enquiries the SHE Team arranged for an external provider who is PASMA (Prefabricated Access Suppliers and Manufacturers Association) trained to examine the scaffolding tower. The Company confirmed the SHE teams finding and therefore two PASMA courses were arranged which would ensure all individuals inspecting scaffolding towers and aircraft staging would be trained to the appropriate level.

The Company have also reviewed its other UK locations and training has been arranged for those personnel involved in this type of activity. The message of the story "Don't ignore your Don't Walk By Schemes".

It would be great to hear similar stories and understand how you maintain a positive reporting culture.

We do have the same Scheme, and it works perfect..because there is lots of management commitment in it...

great idea but as pradash says management must be committed

the last company I worked with who used the system had lots of forms, advertising and QA linked with its DWB system but unfortunately senior management were only concerned with the QA & kudos bit and day 2 day management were concerned with production so the system was really a dead duck

best of luck and try to get management to really buy-in
