Hi I did this course some years ago but have lost my certificate, how can I get a replacement

Depends how long ago you did the course. It may be out of date. In which case you will need to undertake a refresher.

I didn't think it ran out of date.

My certificate has the pass date but no expiry date.

Contact Training Dept at IOSH. They will tell you what to do and what information they require. They are very helpful.
