Rank: Forum user		The issuing of "yellow or Red" cards is down to the company and the format that suits their existing culture best. Personally I believe that these cards should only be used to help develop the safety awareness of an individual and thus enhance the existing culture. These cards should not be used as a discipine for unsafe working practices but more as a tool to help remind and to focus the individual on why the practice was unsafe. Safety professionals are more and more realising that intercommunication skills is a much more effect tool in combatting unsafe practices than the stick. Plus people respect you more if you treat them like adults rather then "bold children". No one is adverse to accepting help, it is the way help is handed out that could lead to the advice not been listened to. Yellow/Red cards? Used as a training aid rather than a disciplinary system. The caveat to that of course is when an individual ignores safety then disciplin is neccessary.

What I am interested in specifically, is in what format people are delivery such"cards"where they have adopted such a system. Andrew Hi Andrew, we have a card system as part of our H&S management system. Ours is a simple word based document which is completed by the issuer and pdf'd. We have Red, Yellow and Green as we reward good safety performance too! RED / YELLOW Info detailed is : Name of person card issued to, Trade contractor involved, Date card issued, Project name and number, Name of the person issuing the card, Location of the incident We then have a list of reasons for issue which are ticked as appropriate: Abusive language/ aggressive behaviour, Inadequate Health and Safety supervision / not enforcing site rules / incorrect equipment supplied/ in use., Incorrect use or erection of mobile towers Misuse of swipe card or not signing on/off site, Smoking on the site except in the designated areas, Not wearing the correct Personal Protective Equipment (PPE) for the task., Consumption of alcohol or drugs., Use of unauthorised access routes / unauthorised access to occupied areas, Use of radio / walkman or similar., Unauthorised removal of building protection., Storage of materials against finished walls., Graffiti and or damage to any part of the works., Blocking emergency escape routes/ poor housekeeping., Unauthorised alterations to scaffolding. , Incorrect/ unsafe use of tools, plant or equipment, Not working to or following the requirements of your method statement / risk assessment / COSHH assessment., Other reason – please specify: Plus a comments box to provide any further specifics.
