Hi Andrew, we have a card system as part of our H&S management system. Ours is a simple word based document which is completed by the issuer and pdf'd.
We have Red, Yellow and Green as we reward good safety performance too!
RED / YELLOW Info detailed is :
Name of person card issued to, Trade contractor involved, Date card issued, Project name and number, Name of the person issuing the card, Location of the incident
We then have a list of reasons for issue which are ticked as appropriate: Abusive language/ aggressive behaviour, Inadequate Health and Safety supervision / not enforcing site rules / incorrect equipment supplied/ in use., Incorrect use or erection of mobile towers
Misuse of swipe card or not signing on/off site, Smoking on the site except in the designated areas, Not wearing the correct Personal Protective Equipment (PPE) for the task., Consumption of alcohol or drugs., Use of unauthorised access routes / unauthorised access to occupied areas, Use of radio / walkman or similar., Unauthorised removal of building protection., Storage of materials against finished walls., Graffiti and or damage to any part of the works., Blocking emergency escape routes/ poor housekeeping., Unauthorised alterations to scaffolding. , Incorrect/ unsafe use of tools, plant or equipment, Not working to or following the requirements of your method statement / risk assessment / COSHH assessment., Other reason – please specify:

GREEN CARD Info:

Offering a practical suggestion to improve an aspect of site health and safety, Taking action to alert site management of a potential hazard, Taking action yourself to avert a situation which could have resulted in injury or damage, You have been nominated by somebody on site for your contribution to achieving high health and safety standards, Promoting environmental awareness and best practice, Constantly working safely and setting a good example to others, Other reason – please specify:
