I don't have a sample to hand but used the format similar to Dupont STOP forms (AFAIR, they originated documenting the STOP system).  Formatted for each activity/ industry or work specific if needed - used some very bland and generic and some quite specific and used for both good and not so good practices.  Lots of images on Google to give an idea.  Using tick boxes and limited writing space makes them easier and so more likely to be completed, also easier to extract stats / highlight good or bad activity.
