How would you ensure you had a robust training programme in place for work equipment, besides looking at training records and accident results

I think you are referring to the effectiveness of work equipment training. My opinion is that you need to look at the training evaluation and how it is applied day to day activities. In that case number of unsafe observations related to work equipment is decreased while increasing safe behaviors.
