I would be curious to know what systems others have in place and in what format they issue the non-conformance (yellow / red card).

The issuing of "yellow or Red" cards is down to the company and the format that suits their existing culture best. Personally I believe that these cards should only be used to help develop the safety awareness of an individual and thus enhance the existing culture. These cards should not be used as a discipine for unsafe working practices but more as a tool to help remind and to focus the individual on why the practice was unsafe. Safety professionals are more and more realising that intercommunication skills is a much more effect tool in combatting unsafe practices than the stick. Plus people respect you more if you treat them like adults rather then "bold children". No one is adverse to accepting help, it is the way help is handed out that could lead to the advice not been listened to.

Yellow/Red cards? Used as a training aid rather than a disciplinary system.

The caveat to that of course is when an individual ignores safety then disciplin is neccessary.

I wuoldn't disagree in what you have said, nor indeeed that a successful safety culture needs to look beyong reward and punishment.

What I am interested in specifically, is in what format people are delivery such"cards"where they have adopted such a system.
