Do you work for the hospital or an agency that works with the hospital? However, whoever you work for should be able to provide you the form/checklist. If you work for yourself, then you should be able to find a pro-forma of the checklist on the internet. 

I agree with Ian.  A hospital is an extremely complex environment. It covers such disparate activities as catering, building maintenance, cleaning, operating theatres, high intensity care units, maternity units, outpatients departments, opticians, hearing, etc., etc., etc.

I find it difficult to imagine a single check list that would adequately cover everything, including sterile operating environments, public access areas, visitors/patients, accident and emergency, hygiene and infection prevention policies, glove use, and so on almost ad infinitum.

Could any one person be adequately informed about every aspect to conduct a risk assessment based on a check list? For me any review needs to be a team approach, involving the safety and health specialists as appropriate, together with those with expertise in the different areas, activities and tasks depending upon which area of the hospital is under review.
