I have a "Health & Safety Report" which you are welcome to pick over.

Although aimed at the college environment it is a Word document so is easily adapable

we use a near miss reporting form that combines both safety and environmental near missess. Unfortunately i don't have an electronic copy.
If you let me have a postal adress i will send you a copy along with the relevant section of our Rules and Procedures Manual.
