Has anyone produced and is willing to share a template which allows accident information to be printed directly onto F2508 forms or has an electronic version of an F2508?

If you order a copy of RIDDOR from HMSO you should also get a floppy disk with the forms on it.

Alternatively, if you can wait until the weekend, I will e-mail a copy from my home computer.

If you use the telephone reporting line you will be faxed the completed 2508 anyway.

I can't remember the last time I filled out a 2508!
