I have been charged with the task of carrying out a perception survey to satisfy three outcomes: 1.A requirement of ISRS 2.Advice from HSE 3.Remedial action from an accident investigation.
It needs to address general loss control/safety issues. I already have some ideas and material to work from, but could do with some help. Also, I could do with some ideas on how to get staff to fill them out honestly and productively. Any help offered would be most appreciated.

Nick,

I had some experince in carrying out perception surveys in behaviour.

Behaviour extends from a number of issues that you need to identify before you dive in.

What is you industry, staff turnover, the recommendations of the accident report, particular areas of cause etc.

If you have these details then I may have the remenants of a system in my noggin somewhere, and a powerpoint presentation to ensure honsety in completing any forms (Its a real art and very satisfying when done properly).
