My company are updating their pre-employment procedures and I would appreciate help in formatting a pre-employment health questionnaire.

Can anyone help as to content or if there is standard forms available?

Pro's and Con's would be of help also.

Who would be assessing the questionairres?

All pre-employment health questionairres should only be interpretted by qualified health personnel i.e. OH nurse/doctor - not by Personnel manager.
The questions asked on the form should be relevant to the job description activities and recognised hazards and any actual screening undertaken pre-employment or on employment should be wholly related to requirements highlighted in risk assessments e.g. audiometry for baseline if regular exposure to noise: lung function if possible exposure to respiratory irritants
