Good Safety Audit Result Means Low Accident Rate?

To most safety practitioner believe that accident rates are outcome / reactive / negative indicator, while safety audits are upstream / proactive / positive indicator on safety management system. Though there SHOULD be close correlation between them, according to some surveys there aren't. How come good safety audit result does not necessarily reflect good accident record? Any factors contributing that failure in the management system?
