If you send your e-mail address to me at my works e-mail address I will send you a copy of an accident reporting and an accident investigating procedure as samples to use for layout. I am on holiday for 2 weeks from 21/07/01 so unless I receive your details before 12-00pm on 20/07/01 it will have to wait. Paul
