I am looking for any reports from the past 5 years or so where individual supervisors or managers have been prosecuted in health & safety cases.
It would be especially useful if anyone could point me towards any cases involving office or light industry environments.
Any assistance would be greatly appreciated.

Not sure of any individual case for managers, although Barrow re: legionnaire comes to mind regarding prosecution?

Have you tried the HSE data base 'name and shame'? Sections 7,8,33 & 36 offences of HSWA would apply, although most are rarely used against employees.

There have been 2 recently - unfortunately I don't have the links at hand, but someone else may provide them?
One on the railways where the supervisor was prosecuted and the other at a chair lift in the Scottish mountains?

The Barrow case seems to be the obvious and high profile choice. I would check the HSE database as suggested above, S7 and S37. There was a case in Norfolk, Thetford way where a couple of people dies after entering a slurry tanker. Also fatality at a waster paper plant in Norfolk, owner/manager convicted, fined and imprisoned.
