Anybody could give advice how should look protocol for taking actions as a result of monitoring arrangements?

I'm not sure what you mean by a protocol? I would write a plan, that says what is wrong, who is going to do what about it and by when.

Perhaps a priority protocol for remedial actions required following an audit or an inspection - yes?
At its simplest then, in terms of timescale:
Category A = immediate, stop until remedied.
Category B = to be attended to within a moderate timescale (measured in days or weeks, might involve a preventative repair, arranging a toolbox talk/refresher, that sort of thing)
Category C = an action to be considered in the longer term as part of business strategy and planning, usually involving significant investment and the preparation of a business case.
You could plot a similar scale in terms of risk.
