How to write this one up? As I finished my shift this morning I went to use the toilet before departing, there are two urinals so decided to use the left one for no particular reason. As I stood minding my own business the overhead strip light detached and swung down on its cable to come to rest illuminating the right hand urinal (looks like I made the light choice there).

Now it is a near miss (the amount of possible puns here!!!) but what would be the point of writing it up. For a start the light fitting has been in place for over five years and has had regular maintenance (how do you test to see that it is still securely fastened to the ceiling apart from obvious signs of looseness?). Obviously the duty electrician has been called out to make the light safe.

I suppose I could recommend hard hats just in case ;-)

Similar thing happened a few years ago when my assistant stood up from his desk just before the strip light swung down across his chair.

We had an immediate visual check of all strip lights [a couple of hundred]to see if they were 'coming away' from the ceilings [A couple were identified and the small 2 man office emptied asap], a warning e-mail to everybody, and a 'full check' by electricians [working through the night] to confirm the fittings were OK.

Such a failure can indicate that similar lights with similar fixings and age could be about to fail

Paul, I think that in this case you may be justified in charging the laundry cost as a business expense!

There will be reasons why the light fitting support failed in that way at that time. Find these out and you may find that others are in a similar state, needing some form of attention.
