Posted By Robert K Lewis Diane, Take5 is a short process based on the steps STOP; Step back and observe THINK through what you are about to do IDENTIFY Hazards (problems) CONTROL hazards and communicate those that cannot be controlled COMPLETE task safely Iyt is issued as an A6 size card at induction Unsafe Act indexing is a bit more complex and involves counting of UA's occurring in a fixed time period, done by line managers on a regular daily basis. These figures with the number of counts and the no of staff can give an index of UAS perman per standard count I am preparing a paper on it but ring me for more info Bob
