I think I've recently seen a press release or news article relatively recently about a particular scenario, but I can't now find it. Does this ring any bells:
There was an excavation, it was (apparently) properly supported, but then before it was to be filled in the supports were removed. At this point someone noticed that a surveying staff had been left in the bottom, and they entered the excavation 'just quickly' to recover it, and it collapsed. I don't recall if they were killed or injured.
Does anyone recall this, or can point me to an article or press release about it? I'm wanting to cite it and my internet searching is letting me down.
