I have a "Health & Safety Report" which you are welcome to pick over.
Although aimed at the college environment it is a Word document so is easily adapable

I am also looking for some guidance on Near miss. I would like to bave a book or notes clearly defining near misses, reporting and related concept.
I would be grateful if anyone provide me the necessary guidance in the matter

we use a near miss reporting form that combines both safety and environmental near missess. Unfortunately i don't have an electronic copy.
If you let me have a postal adress i will send you a copy along with the relevant section of our Rules and Procedures Manual.
