Have any of you been unfortunate enough to be involved in an HSE investigation following a fatality? If so, I would really appreciate any advice/info you could share with me regarding what might be expected, what questions might be asked and how an organisation can best be prepared.

Obviously I can say very little about the incident, but it involved sub-contractors.

Any tips would be gratefully received.

is a training resource for LA health and safety enforcement officers.
It sets out the way accidents are investigated using a very realistic example -and while the example isn't a fatality, it's close enough.

I appreciate that you may not find yourself with much time on your hands at the moment - but this could give you an insight into the how they'll approach things.

Not a fatality...but a near one. It also involved contractors.
