I am drafting guidance notes for issue to managers regarding accident investigations and i want to take into account the proposals set out in the recent consultative document about different levels of investigation depending on the scale and nature of the event. I am thinking of including a structure of 3 levels of investigation, Simple, standard and eleborate.
Has anyone experience if this type of approach has worked for them.

Keith,
We use 4 levels of investigation, depending on POTENTIAL severity of the accident.
Level 1: Investigated by Supervisor, Reviewed by Line Manager & Safety Advisor
Level 2: Investigated by Supervisor and Line Manager, Reviewed by Senior Manager and Safety Advisor
Level 3 & 4: Panel of Investigation formed.
It does work quite well for us.
