I guess you want to introduce near-miss reporting. The real crux of the problem is to get people to recognise the true near miss/hit. It is often perceived when something happens in which somebody narrowly avoids injury. The simple recording of these events and the corrective actions, and their circulation, misses out on the vaste reservoir of potential injury. We are also in the area of Unsafe Acts and Unsafe Conditions. If a ladder is left unattended leaning against a wall is this a Near Miss? - My answer is yes but at root it is also an unsafe act because someone has placed it in that position and left it, others may simply call it an unsafe condition.
