Has anyone ever implimented, a sixty second risk assessment for maintenance staff in an industrial environment, were there any improvements? Does anyone know where you could get a simple check list that could be utilised. I understand this wouldnt replace a full blown risk assessment but I believe if we could get the maintenanace staff to go through a short checklist/decision tree before starting the job it may reduce some of the accidents.

Yes I call it TAKE 5 and if you e-mail me I will happily discuss it further. It is also intended as a supervisory lead in if they see something wrong.

Darren,
Three questions should start the process off and depending on the answers, direct workers to the next step.
. What can cause harm?
. What are you doing about it?
. Is it enough?
If it is not enough, rationally they should look for what needs to be done to safeguard the operation.
A short course on managing this process is available (free) on-line at our website, http://www.confinedspaces.com/oac/index.htm
I hope this is useful,
Regards, Philip
