I am looking for a simple risk Assessment format. can anybody give me a set of simple forms that they have used for industrial Risk Assessment?

You will find materials and guidance risk assessments on www.web-safety.com under "Free Safety Manual". There are forms on this section too that you can download in word format.
A free on-line training program on the OAC at www.confinedspaces.com entitled "Work Safety- Manage Safely" (you can find it in the training section). This program is a step by step guide to operational control.
Both of these should start you on you way,
