I have been asked to introduce an audit system and am not sure where to start. I know what an audit is and I know the basic principles of how to conduct an audit, but i work for a large Local Authority, so the audit system needs to be comprehensive.
Has anyone developed such a system. Or does anyone use any external auditing arrangements or bespoke audit packages.

I also audit within my own LA. Don't over complicate the issue and if it's a large organisation break it down and pick on topics. If you email me I can give you our audit template that we use for assessing managements systems of each of our service units and schools. Ownership of safety belongs to them not me, I just see if they are doing it. My office email is a.byers@poole.gov.uk just remind me to send you our template. It's not big, it's a prompt that makes you dig deeper depending on the response you get. They should be monitoring and inspecting their own areas, you check to see it's happening

Start off with the basic philosophy that an audit is a quality / monitoring tool - you are merely verifying their compliance. Next, work up from the basics and do not overcomplicate things. Do not rush to introduce great reams of paperwork if all you need is a basic check system.
Be specific about what it is you need to audit, break this down in to topics and identify the key issues that you need to verify. Do not forget that they can have all the paperwork and procedures in the world, but you will need to verify its effectiveness by talking with the staff / walking around the facilities. Remember that you are trying to validate if these procedures are ac tually used and hence is the workplace made safer by them.

An audit was done of my dept before I arrived & we now have a comprehensive action plan based on 18001/2. Further audits have to be done so I'll be going down the same track. E-mail me if you want more info.

A bit of name-dropping coming up...
Andy Gay and Nick New who wrote HSG65 once told me that they don't really approve of any commercial audit systems. Just go through HSG65 and try to turn all its points into questions, was what they said. You really are better off developing your own system appropriate for the organisation.

As a BSI 9000/14000 Internal Auditor for my Company I always start with the simple concept"Say what you do/do what you say/
