Has anyone developed a questionnaire/ survey for supervisory staff?

Covering areas like:

Do they know the role of the Safety Representative?

Their level of Safety & Health Knowledge?

Have you seen 'Supervising for Safety in Woodworking' from HSE in the UK? It might be a good starting point. It can be downloaded from their website.
