I am trying to provide some simple guidance for managers / supervisors to enable them to complete an accident / incident investigation form.

Part of the form requires them to identify the immediate cause (what unsafe act or condition caused the event) and the secondary cause (what human, organisational or job factors caused the event).

They are having difficulty understanding this section and I require a concise and easily understandable explanation to include in the guidance.

All help gratefully accepted.

We work on the basic of 5 why's eg

lets look at a cut to the hand

Why did the accident happen?

Because he put his hand in swarf without gloves.

Why did he put his hand in the swarf without gloves?

Because there were no gloves available.

Why were there no gloves available?
Because these have not been specified for the job.

Why have these not been specified?

Because no risk assessment has been carried out identifying the problem.

Why has no risk assessment been carried out?

Lack of management control.


Therefore, if you keep asking why then eventually you should come up with a root cause for the accident. So the immediate cause is obvious, no gloves, the root cause is a lack of management control.
