I have to prepare an accident improvement plan. Could anyone send me a sample format?
Many thanks.

I can't resist asking which part of an accident your plan is aiming to improve? Seriously, though, you probably need to give us some more background on this so we can know exactly what you do and what it is you're after.

Have read Allan's reply and agree perhaps a little more info needed. I presume you mean action taken after accident to minimise future incidents?

We have just introduced DATIX, which is a a d/base that links, accidents/incidents with claims and litigation and eventually will feed this info into our Risk Register.

There's a huge amount of basic "accident form completion" training going on at the moment + we are piloting a "grading" system whereby incidents will be graded when they are inputted and then regraded once they have been investigated and actions put in place. I won't ramble further but if this is the stuff you're after, let me know.
