Could anybody point me in the right direction OR email me a template/sample of a LEV test report. Before I take the measurements I would like to see how the findings should be portrayed.
Any information would be appreciated.

As with most things, there is no pro forma for LEV reports. The COSHH ACoP specifies what should be included in such a report but not how it should be arranged. In addition, a schematic diagram is very useful for identifying the components of the system and the location of measurement points.
I find it useful to break records into two parts: the non-changing information (location of the system, intended performance, etc.) and then the results of the examinations and tests.

You also need to make it clear what you measured, where the measurements were taken, and what equipment was being used to make the measurements - you may need to produce a dimensioned sketch with sampling points.
Pass/fail criteria are also quite important - it is a real help to the owner yo know whether all those nice numbers on your report mean that everything is OK or not.

Refer to "Maintenance, examination and testing of local exhaust ventilation"-HSE Guidance HS(G)54. It has in Appendix 1 -information required of an LEV system-a pro forma as an example of recording the data required for monitoring an LEV system.
