Posted By Des I am preparing a presentation on incentive schemes. I have come across some that can only be described as crazy and often only help to supress incident reporting. I am looking for some more examples. I do not want names (of companies or individuals) as it is not meant to embarrass anyone but will be used to try and educate others of the pitfalls. Any that you have personally witnessed/experienced in the past would be helpful.

In my limited experience any incentive scheme that financially rewards individuals to lower accident statistics can turn on itself.
A true example is a individual who was injured on one partcular site was bundled in the back of a works van and taken off site before he could report it. The reason for this action being that the squad he worked in would have lost their bonus paid every two "injury free" weeks.
