I am in need of a good workshop exercise for a forthcoming permit to work training session.

there are a number of 'exercises' that you could consider... A few ideas:
1) hot work in maintenance shop - fire by a number of causes arising - explosions, and long term exposure to welding gases ect..
2) work in confined spaces - doing hot work and carrying out a number of other activities or use of substances that may be toxic or poisonous + atmosphere problems etc through lack of/enriched oxygen ect
3) PUWER/LOLER, pressurised systems and lifting operations where correct procedures have not been followed for inspection of lifting equipment/tackle results in fall of items being lifted, escapes of gas, oil under pressure, fire/explosions ect
4) Maintenance operations requiring works which need excavations or breaking into walls ect and contacting live services that should have been located/isolated under PTW
5) Electrical works on PTW, failure to isolate, wrong systems identified through failure to test and locate, secondry feeds not known about or checked for, live working and isolation/insulation...
6) Temporary working at height for maintenance operations under a PTW, risks associated with fall protection system not inspected/tested, failure of temporary platforms, ladders, number of persons involved, ect etc
7) mechanical systems and plant operations and maintenance under a PTW, starting up unexpectedly of machines and plant, isolation, incorrect use/type etc etc...
