I used it in a large office environment.
Several members of staff complained about the wording of the questions, asking that we made them more appropriate to their work activities.

Not much direct experience myself but you could try searching the research reports on the HSE web site for a review of feedback. Otherwise try contacting Norman Byrom at HSE in Bootle. I believe he developed the tool and I think he did some factfinding to get feedback on it.

I work for the Environment Agency. We have used it both selectively within our manual workforce and across the Agency. I would be happy to give you some feedback if you want to give me a call 01132312477.
It's a tool you really want to be able to use several times. First time to identify the weaknesses in what you've got and then periodically (say annually) to identify the effects of what you put in place to improve.
