Posted By fats van den raad Don't really know how I got roped into this, suppose I was too slow to duck!!! My co. is looking for a triplicate type disciplinary pro-forma that would be completed at the outcome of a disciplinary hearing, sifned by all involved and then one copy for the culprit, one for the personnel file and one to stay in the book. Anybody have such a form that I could have a look at for some ideas to formulate our own in-house one?? Appreciated in advance PS reminder to self: Duck next time, idiot!!!!!
