The only reason I don't offer you a 'template' is that I don't know enough about the physical and social environment of your business.
The soundest way to get to grips with this matter is probably is use the BS 8300 standard and associated guidance.
Jordan's have published a good guide in 2000 by the leading disabillity barrister, Brian Doyle

I am a H&S consultant and also a DDA auditor by trade. It is difficult to offer a template as such as unlike H&S, DDA is not black and white, there is no right and wrong, it all comes back to what is reasonable and this takes some skill and experience to decide. if you want to contact me directly for further advice please do so on 01604 646480.
