This is a long shot but maybe someone might remember a case in the last 10 years where a female employee banged her leg against a piece of office furniture, developed a DVT and died as a consequence. The local LA, I believe North West England/ North Wales somewhere successfully prosecuted the company under I think the Workplace HSW Regs. I can't be any more specific than this but hopefully someone can help.

Well as you asked, Sean the facts are -
The incident fits one that we had about 6 years ago when we had an office at St David's Park Ewloe.
A female employee accidentally hit her leg on her own desk. A week or two later she collapsed and died at home. The post mortem showed a deep vein thrombosis leading to heart failure. HSE investigated along with ourselves and our insurers. HSE took no action against us not even any off the record advice. The coroners inquest found accidental death linked to an injury at work (can't remember the exact phrase)
