Has anyone been involved with the design of DIRO bus stations or have knowledge of any accidents at them?

I believe there was an incident in Banbury a few years ago when someone was killed by a reversing bus.
Now the bus station has to employ a permanent banksman when an bus is reversing from it's parking bay

I know that Huddersfield has a DIRO design, memory seems to ring a bell that a fatality occurred there - best sourcing Huddersfield Daily Examiner for press releases. Hope this helps..
