Would appreciate any help / advice or sample permits to assist me with my latest H&S project at Work - Contractors Guidelines, Hand book, and last but not least Permits to Work.
I would like clarification on what should be issued when i.e. Would it be a Clearance Certificate / General Work Permit or another form, for a contractor who provides `Plant Maintenance` ( the green leaf kind.)

The reason for asking is that we have general contractors, cleaners, catering etc... then we have others who are maintenance issues, photocopiers, security system, repair of gymnasium equipment, etc...

If any one has the time to assist, I would be grately appreciative.

The Fire Potection Association have a document 'Recommendations for Hot Work' (RC7) which includes a sample permit which was produced jointly with the Association of British Insurers.
