Terms of Appointment/Conditions of Contract etc

Dear All
Having recently started up as a H&S consultant I was wondering if anyone would be prepared to share with me a copy of their Conditions of Engagement or Contract Conditions etc for use when providing consultancy/advisory services to my clients.
I have prepared my own, but not being an expert in this area, and not knowing of any standard forms of appointment etc (which seem to be available for most other professions) I was hoping that some kind IOSH member would allow me to compare theirs with my own.

IOSH can provide terms and conditions but interestingly with 9 years as a consultant we've never been asked for or provided any terms and conditions. We quote for the job, state our terms eg payment within 30 days, quote valid for 30 days, and they take it or leave it.

We also just give general commercial terms (pay up within 30 days or be cursed by our wicca consultant) which customers ritually ignore in order to impose their own commercial terms (give us a 10% discount if we pay within the next two years)
One thing which we explicitly exclude is "results". We cannot gaurantee that LTI rate will fall by "30%" within "12" months. There are too many variables.
However, we have heard of consultants contracting to be paid a percentage of cost savings over a set period. This could be a fun thing as I know that some of our clients have calculated savings to be in the region of millions of pounds within five years of the intervention !
Mind you, have not heard of consultants agreeing to pay up if the results dont get better, or even get worse.
