Posted By Guy Anderson We ar a SME approx 50 employees. We are looking to improve the safety culture and my first step is to assess the employee perceptions of the safety culture within the company. I think the initial step would be to issue a questionnaire. Does anybody have or know where a general type of questionnaire can be 'obtained; or have other suggestions on how to proceed?

In relation to what you call a 'general type of questionnaire', may I encourage you to exercise some caution.
To the extent that your employee population is relatively sophisticated, and depending on what you want to achieve by gathering survey data, it is worth using a tool that reflects hazards that pose critical risks to your staff.
In this light, do consider using repertory grids and a Q sort, in addition to or as well as a 'general type of questionnaire.

Posted By Ian Waldram Have you read the IOSH 'Position Statement' on H&S Culture/Climate? (see under Technical information - upper left. The 'Respect for People' toolkit referenced there is a well-validated starter kit. Although developed for the construction sector, most of it seems applicable more widely - the questionnaires are easy to copy, complete and analyse, and also they link H&S with the general culture in the organisation and how well employee potential is used.
