I've used repertory grids to measure the effects of interventions on performance in relation to occupational stress.
To the extent that the employees involved, and to some extent their managers too, engaged with the process, using the grids made significant differences in their willingness to experiment and to listen rather than remain in relatively comfortable but unproductive patterns of thinking and acting.

Posted By Ian Waldram Have you looked at the IOSH guidance on Reporting Performance? (available in Technical information, upper left). Lots of ideas, not all validated though. Also you could try sections from the HSE 'CHASPI - Corporate H&S Performance Index' system, available on their website. That is still being validated, but again is full of ideas.
In both cases, the emphasis is on turning qualitiative data into something numerical and reliable. Another approach is the 'Culture Maturity Model' referenced in the IOSH Position Statement on Culture, which is more qualitative.
