Posted By Eric Burt Our organisation is considering the ways in which we can share accident data with Trade Union Safety Representatives following the withdrawl of the Accident Book earlier this year. Previously, Safety Reps would inspect a premises and glance at the accident book. Due to Data Protection they can't do this any more. We have devised our own accident form which complies with DWP requirements (we have received approval from them) and these are held confidentially once they have been completed. Hence Safety Reps are not receiving the amount of information that they received previously. I understand that there is now a box on the new DWP / HSE accident form (not the F2508) which the IP can tick to say that they consent to the information being shared with their Trade Union. Anyone aware of this ? - I have loooked at the form on Barbour Index but it hasn't been changed on their database. Regards, Eric

Posted By Debbie Walker The phrase you're looking for in the new accident book reads as follows... "By ticking this box I give my consent to my employer to disclose my personal information and details of the accident which appear on this form to safety representatives and representatives of employee safety for them to carry out the health and safety functions given to them by law." There is then a space for them to sign and date it.

Posted By Pat Burns MIOSH - SpDipEM - AMIQA I am not sure how others have found this new book but I have had to make an additional page to gather as much informaton as I need on receipt of the page submitted as it could be coming from an outstation. The first thing I noticed is that the date of birth was not included on the new form and going by what I have read on this thread other information which must now be seen as relevant was also missing. It appears to me that a quick fix was made to the book to comply with data protection without much thought for those who have to use it. Despite a subscription to HSE Direct and updates from croner etc I am not aware of the amendment made to the book. Perhaps HSE Books is worried we might all return them and ask for a refund to purchase the updated version.
