I am a health and Safety Advisor for a global company. After suffering many serious accidents, some fatal the company have assembled a task force to achieve zero accidents.
They have asked for my input to the group on how we can address the issue.
I have many ideas, but would like input from my fellow safety professionals.
I get the impression that the task force are not H&S professionals, but executive board members who tend to think that all accidents are caused by unsafe people.
I appreciate this is not a simple question and there are no magic answers

I believe setting a target of zero accidents is unreasonable. Any target must be achievable and if it is not achievable then people will become disinterested and disheartened.
Non H&S people, and especially those in Director/Executive positions often have a 'Knee Jerk' reaction to fatal or very serious accidents at work. What you need to do is get them to jerk their knees in the right way.
Look at your accident statistics over the past couple of years, identify any trends then identify where possible, what causes were attributable to the accidents.
Based on this information then identify what corrective and preventive action programme(s) you need to develop and implement.
These could be a multitude of things such as risk assessment, employee training and awareness, mechanical and engineering controls and almost certainly a level of management responsibility and accountability which can be measured and linked to overall performance.
