Posted By CU I am trying to develop a safety card to take into account all actions good or bad. The current card only looks at the negative side of things like unsafe acts etc. I would like to be able to develop a card where individuals can be credited for good H&S practices. does anyone have a format that I can have for ideas. Or is there a different approach to take rather than safety cards.

Posted By Kevin Moran TSM Reporting safe Acts can be achieved through the DuPont STOP System - it is good to acknowledge safe acts but I just wonder how much effort you want the workforce to put into this and for what benefit - Reporting unsafe acts and conditions help to trend and prevent accidents, but safe acts should be the norm - I agree we should recognise a reward the people that go the extra mile, but report cards for this seems a bit time consuming...

Posted By garyh I use an "observation system" - the report card has a list of areas of observation which could be "Safe" or "At risk". Areas include PPE, Posture..........you know the drill. If safe, the "Safe" box is ticked , if "At risk" then this is ticked (both boxes are next to each criterion). Of course, very few "At risks" get ticked - this way you accentuate both +ve and -ve. The important thing is to USE THIS DATA to analyse what is happening, and to give feedback.

Posted By Frank Hallett Perhaps you might like to consider how to turn the negative [unsafe event] into a positive [safer] event by identifying the negative but also requiring the positive act of the event identifier to be recognised as a culture reinforcer? If a near-miss is reported; include what the reporter did AT THE TIME to make the situation safer. As identified in the earlier responses, you should really ensure clarity as to the function of the Reporting Card system first. Frank Hallett
