Hi
Does anyone know where I can find reports on the 21 accidents and 5 deaths reported by the HSE in the last 5 years?
Does anyone think that a 1ft high curb stone surrounding a grave presents a serious and immediate risk of falling and crushing someone?

The risk will depend on the other dimensions of the curb and how much is buried in the ground. If it is only 12" high, foot injuries are likely to be the worst outcome if it does fall over.

Nice answer Gareth; bet the HSE can't answer the question as fully as necessary though.
Regarding the potential for the stone "falling & crushing"; it will depend greatly on the activities being carried out in the vicinity. There is a theoretical possibility of the stone doing just that, but from a real and pragmatic point of view, only someone working below the stone is likely to be threatened by this eventuality.
Go to the HSE web-site and read the section on "Realistic Risk Management and Enforcement". {Yeah, I know that isn't exactly what it's entitled - but I don't have a photographic memeory]

This has all come about thanks to european friends, mainly the Germans. A device called a topple tester has been produced to test grave stones, it basically gives the stone a good hard shove ! Fine far German grave stones which are held in place by steel pins not so good for British grave stones which tend to be cemented into place. After flattening many a cemetry some one realised the difference. We test them by hand, if they can be easily moved by hand they need repair.
